Gracewell of Bath has installed an innovative care device that tracks individual resident care plans to ensure they receive the best support according to their personal preferences and needs.
CareConnect electronically tracks the care being delivered to each resident, with team members inputting real-time updates and having secure access to individual resident profiles.
Using the privacy-protected, touchscreen kiosks available at the home, team members will have immediate access to the information they need to continue to deliver high-quality, tailored care to every resident.
The advanced software can record many distinct aspects of a resident’s life at the home, from monitoring changes to health conditions, to recording residents’ personal preferences, including reminders of a favourite dessert or an activity which they enjoy.
The system provides an opportunity for carers to ensure care programmes are maximised, and that residents can lead fulfilling, engaged and enriched lives.
CareConnect, which can only be accessed by authorised members of the care team when at the home, allows the care being delivered to be adjusted accordingly, so that residents continue to receive the best possible support, when they need it.
The launch of Gracewell CareConnect in the UK is set to further enhance the communication and coordination across care teams to deliver the best personalised care for residents.
